IMH’s program is designed to support patients with type 1 diabetes, type 2 diabetes, gestational diabetes, and prediabetes through a personalized, evidence-based approach.
Family Medicine physicians act as the primary coordinators of care, ensuring continuity, regular follow-ups, and seamless referrals to endocrinology, cardiology, nephrology, ophthalmology, and dietetics when required. This integrated model ensures that diabetes-related complications are detected early and managed proactively.
The program begins with a comprehensive assessment, including blood glucose profiling, HbA1c, lipid levels, kidney function tests, blood pressure evaluation, BMI assessment, and lifestyle review. Based on this, an individualized management plan is created covering medication optimization, nutrition planning, physical activity guidance, and self-monitoring education.
Medication management includes oral anti-diabetic agents, injectable therapies, and insulin when required, with close monitoring to minimize hypoglycemia and side effects. Patients receive education on blood glucose monitoring, injection techniques, sick-day rules, and recognizing warning signs of complications.
Lifestyle intervention is a core pillar of the program. Dietitians work closely with Family Medicine physicians to develop culturally appropriate meal plans that are sustainable and effective. Physical activity recommendations are tailored to age, fitness level, and comorbidities.
