Cataract Surgery (Phacoemulsification)

Cataract surgery using phacoemulsification is the gold-standard surgical treatment for cataracts and one of the most commonly performed and successful procedures in modern medicine. The aim of cataract surgery is to remove the eye’s clouded natural lens and replace it with a clear artificial intraocular lens (IOL), thereby restoring vision, improving contrast sensitivity, and enhancing overall visual quality. Phacoemulsification has largely replaced older surgical techniques due to its minimally invasive nature, faster recovery, and excellent visual outcomes.

In phacoemulsification, the surgeon makes a very small incision at the edge of the cornea. Through this incision, an ultrasonic probe is inserted that emits high-frequency sound waves to break the cloudy lens into microscopic fragments. These fragments are then gently suctioned out, leaving the lens capsule intact. Once the cataract is removed, a foldable intraocular lens is implanted into the same capsule, where it unfolds and remains permanently in place.

One of the key advantages of phaco surgery is that the incision is self-sealing and usually does not require sutures. This reduces postoperative inflammation, lowers the risk of infection, and promotes faster healing. The procedure is typically performed under local or topical anesthesia and is painless. Most patients are discharged the same day and resume routine activities within a short period, with vision improving significantly within days.

Modern cataract surgery is no longer limited to restoring clarity alone. Advances in intraocular lens technology allow simultaneous correction of refractive errors such as myopia, hyperopia, astigmatism, and presbyopia. Patients can choose from monofocal lenses for distance vision, toric lenses for astigmatism correction, and multifocal or extended depth-of-focus lenses that reduce dependence on glasses. Preoperative biometric measurements and careful surgical planning are essential to achieving optimal visual outcomes.

Cataract surgery is recommended when visual impairment interferes with daily activities such as reading, driving, or working. Delaying surgery does not usually cause harm, but advanced cataracts can become harder to remove and may increase surgical complexity. A comprehensive preoperative evaluation ensures the eye is healthy enough for surgery and helps identify coexisting conditions such as glaucoma or retinal disease.
