This program targets individuals with risk factors such as obesity, diabetes, hypertension, high cholesterol, sedentary lifestyle, smoking, stress, and family history of heart disease. Early identification allows for timely intervention before irreversible damage occurs.
The program includes comprehensive cardiovascular and metabolic screening, including blood pressure, lipid profile, blood glucose, BMI, waist circumference, ECG, and risk scoring. Based on results, patients are stratified into risk categories with personalized care plans.
Family Medicine physicians coordinate care with cardiology and endocrinology as needed, while maintaining overall continuity. Lifestyle modification forms the foundation of risk reduction, supported by dietetic counseling, physical activity planning, and behavioral change support.
Medication therapy is introduced when required to manage cholesterol, blood pressure, or blood sugar levels. Preventive therapies are regularly reviewed to ensure effectiveness and safety.
